How to Prepare for Your Appointment

If you have an upcoming appointment there are a few things you can do to help things go smoothly. If you’re a new patient start by filling out a records release; which you can download right from our website, which allows us to review anything they may have been following you for.
When you come to your appointment try and show up a few minutes early and bring a copy of your insurance card with you. This allows us to make sure all your information is up to date and to look up your insurance benefits, if applicable.

A technician is going to gather medical and eye history from you for the doctor. Things they’re going to ask you include:

  • Surgeries (both eye and systemic)
  • Ongoing systemic medical conditions
  • History of eye conditions
  • Current medications
  • Drug allergies
  • Family medical history (both eye and systemic)

To help the doctor best treat you make sure to bring in your current glasses and if you wear contact lenses to bring the prescription with you.
